The treatment in HIV centres is open for foreigners under the conditions set by the Czech law (you need either health insurance - public or private, but private typically does not cover HIV treatment - or have to pay for the treatment). The fact that you do not speak Czech should not lead to denial of medical services. I would expect to find an English speaking doctor in all HIV centres. On the other hand, some doctors may not speak English and you may need an interpreter in order to communicate with them. I would suggest to clarify this before going to the HIV centre and ask them to preferably assign you to an English speaker.
