Good evening!
Tell me please, I’m interested in the question: if my partner and I are both HIV+, both on ART (antiretroviral therapy) and the viral load is not determined, we are in a monogamous relationship and there are no STDs, can I have unprotected sex with him?
Thank you for your reply!

The main risk in unprotected sex for seropositive couple is a transmission of other STIs, including the risk of HCV and LGV transmission in unprotected anal sex.
If you are checked for them and you are monogamous, this risk is negligible.
There is another one, small risk like antiretroviral resistence transmission. If one of you will develope resistence and you both use the same (or similar) therapy, you can transmit it.
I would say if we evaluate the benefits and losts I believe the unprotected sex bring more benefits for most couples.
